Summary

NSAIDs can blunt the blood pressure lowering effect of an angiotensin receptor blocker and thiazide combination. In combination with a diuretic and an ARB, an NSAID also raises the risk of acute kidney injury, especially in elderly or volume-depleted patients, and may raise potassium.

What this interaction means in plain words

Eprosartan-hydrochlorothiazide and Meloxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Meloxicam belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAIDs inhibit renal prostaglandin synthesis, causing sodium and water retention and reduced afferent arteriolar dilation, which counteracts antihypertensive effects and reduces glomerular filtration when angiotensin II blockade and diuresis are present.

What to do

Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic such as paracetamol where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
