Summary
Tolmetin can reduce the blood pressure lowering effect of eprosartan and hydrochlorothiazide. The combination of an NSAID, an ARB and a diuretic increases the risk of acute kidney injury and hyperkalemia, particularly in older or volume-depleted patients.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion and sodium excretion, opposing the antihypertensive effect and adding to ARB-related renal hemodynamic stress.
What to do
Monitor blood pressure, serum creatinine and potassium within the first weeks of co-therapy. Avoid prolonged use and ensure adequate hydration, or switch to a non-NSAID analgesic.
What to ask your pharmacist
