Summary

Tolvaptan causes aquaresis and, together with hydrochlorothiazide, can lead to excessive fluid loss, hypotension and electrolyte disturbance. Potassium may rise with the ARB, and serum sodium may correct too rapidly.

What this interaction means in plain words

Eprosartan-hydrochlorothiazide and Tolvaptan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Tolvaptan belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic diuresis from vasopressin V2 antagonism and thiazide action, with ARB-related potassium retention.

What to do

Monitor serum sodium, potassium, volume status and blood pressure closely, especially at initiation. Adjust or reduce the thiazide dose if volume depletion occurs.
