Summary

Erythromycin inhibits CYP3A4 and P-glycoprotein and can raise pazopanib concentrations. Higher exposure increases the risk of hepatotoxicity, hypertension and QT prolongation, which pazopanib itself can cause.

What this interaction means in plain words

Erythromycin and Pazopanib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Erythromycin belongs to Anti Bacterial; Pazopanib belongs to Oncology.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated metabolism of pazopanib by erythromycin.

What to do

Prefer a non-interacting antibiotic such as one without CYP3A4 inhibition. If unavoidable, reduce the pazopanib dose per labeling and monitor liver tests, blood pressure and ECG.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
