Summary

Erythromycin prolongs the QT interval and can cause torsades de pointes. Salbutamol may add to this risk, especially at high doses or with hypokalemia. Erythromycin may also modestly raise beta-agonist exposure.

What this interaction means in plain words

Erythromycin base and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive QT prolongation, with beta-agonist-induced hypokalemia increasing proarrhythmic risk.

What to do

Check potassium and magnesium and consider an ECG in patients with other QT risk factors. Prefer a non-QT-prolonging antibiotic when feasible.
