Summary

Erythromycin prolongs the QT interval and inhibits CYP3A4, the main route of quinidine metabolism. This raises quinidine levels and increases the risk of torsades de pointes.

What this interaction means in plain words

Erythromycin base and Quinidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive QT prolongation plus CYP3A4 inhibition reducing quinidine clearance.

What to do

Avoid the combination and choose a non-macrolide antibiotic such as one without QT effects. If it cannot be avoided, monitor ECG and quinidine levels.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
