Summary
Mitotane is a strong CYP3A4 inducer that can substantially lower circulating ethinyl estradiol and norgestimate-derived progestin concentrations. This may lead to contraceptive failure and breakthrough bleeding, and the induction persists for a long time after mitotane is stopped.
What this interaction means in plain words
Ethinyl estradiol-norgestimate and Mitotane can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ethinyl estradiol-norgestimate belongs to Endocrine; Mitotane belongs to Oncology.
Why it happens
Induction of hepatic CYP3A4 increases metabolism of the estrogen and progestin components.
What to do
Use a non-hormonal or non-CYP3A4-dependent contraceptive method such as a copper IUD instead of combined oral contraceptives. Continue the alternative method for months after mitotane is discontinued.
