Summary
Iodinated contrast media can cause acute kidney injury, and impaired renal function reduces metformin clearance. Accumulated metformin can lead to lactic acidosis, a rare but serious complication. Ethiodized oil is used mainly for lymphography and hysterosalpingography or intra-arterial chemoembolization, so systemic exposure and renal risk depend on the route and procedure.
What this interaction means in plain words
Ethiodized oil and Saxagliptin-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Contrast-associated reduction in renal function lowers metformin elimination, raising the risk of metformin accumulation and lactic acidosis.
What to do
Check eGFR before the procedure and withhold metformin in patients with eGFR below 30, with acute kidney injury, or undergoing intra-arterial studies with renal exposure. Re-assess renal function about 48 hours afterwards and restart metformin only if it is stable.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
