Summary

Combining a systemic corticosteroid with an NSAID raises the risk of gastric and duodenal ulceration and gastrointestinal bleeding beyond that of either drug alone. The risk is greater in older patients, with high doses, with prolonged use, or when anticoagulants are also given.

What this interaction means in plain words

Etodolac and Hydrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Etodolac belongs to Musculoskeletal; Hydrocortisone belongs to Endocrine.

Evidence level: High

Why it happens

Additive gastrointestinal mucosal injury: NSAIDs inhibit protective prostaglandin synthesis while corticosteroids impair mucosal healing.

What to do

Use the lowest effective doses for the shortest duration and consider adding a proton pump inhibitor in at-risk patients. Counsel patients to report black stools, hematemesis or epigastric pain.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
