Summary

Etrasimod is a sphingosine-1-phosphate receptor modulator that lowers circulating lymphocyte counts, and its labeling cautions against combining it with antineoplastic or immunomodulating drugs. Adding ipilimumab, which causes immune activation and often requires immunosuppressive treatment for toxicity, may lead to unpredictable immune effects and infection risk. Lymphocyte effects persist for weeks after etrasimod is stopped.

What this interaction means in plain words

Etrasimod and Ipilimumab can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Etrasimod belongs to Gastrointestinal Tract; Ipilimumab belongs to Oncology.

Evidence level: Low

Why it happens

Additive immunomodulatory effects: etrasimod sequesters lymphocytes in lymphoid tissue while ipilimumab removes T-cell checkpoint inhibition.

What to do

Avoid combining where an alternative exists, including for several weeks after stopping etrasimod. If unavoidable, monitor for infection and immune-related adverse events and check lymphocyte counts.
