Summary
Etrasimod is a sphingosine-1-phosphate receptor modulator that lowers circulating lymphocytes. Combined with melphalan, immunosuppression and infection risk may be additive, and this combination has not been studied.
What this interaction means in plain words
Etrasimod and Melphalan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Etrasimod belongs to Gastrointestinal Tract; Melphalan belongs to Oncology.
Why it happens
Additive immunosuppression from S1P-mediated lymphocyte sequestration and melphalan myelosuppression.
What to do
Avoid combining where possible, allowing for the prolonged immune effects after etrasimod is stopped. If used together, monitor blood counts and infections.
