Summary

Cannabinoids such as cannabidiol may inhibit CYP3A4 and could raise felodipine concentrations. This may lead to stronger blood pressure lowering, flushing, headache or peripheral edema. Cannabis smoking also causes tachycardia and postural hypotension, which may add to the effect.

What this interaction means in plain words

Felodipine and Marijuana have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Cannabinoids (notably cannabidiol) may weakly inhibit CYP3A4-mediated felodipine metabolism, with additive hemodynamic effects.

What to do

Monitor blood pressure and heart rate in patients who use cannabis regularly and counsel on dizziness on standing. Reduce the felodipine dose if hypotension or edema develops.

What to ask your pharmacist
