What this interaction means in plain words

Felodipine and Nirmatrelvir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Felodipine belongs to Cardiovascular; Nirmatrelvir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Ritonavir in the nirmatrelvir combination inhibits CYP3A4, reducing felodipine metabolism.

What to do

Consider a temporary felodipine dose reduction or hold during treatment, and monitor blood pressure. Resume the usual dose a few days after the course ends.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
