Summary

Mesalamine can rarely cause interstitial nephritis and NSAIDs such as fenbufen can reduce renal perfusion and cause kidney injury. Concurrent use may increase the chance of renal impairment.

What this interaction means in plain words

Fenbufen and Mesalamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive nephrotoxicity, with NSAID inhibition of renal prostaglandins combined with the intrinsic tubulointerstitial toxicity of mesalamine.

What to do

Monitor serum creatinine and urine output at baseline and periodically, and avoid the NSAID in patients with reduced renal function. Prefer paracetamol for analgesia when possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
