Summary
Sparsentan lowers glomerular pressure through endothelin and angiotensin receptor blockade, and NSAIDs constrict afferent arterioles. Combined use can worsen kidney function, particularly in volume-depleted or older patients, and may blunt blood pressure control.
What this interaction means in plain words
Fenoprofen and Sparsentan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fenoprofen belongs to Musculoskeletal; Sparsentan belongs to Genitourinary.
Why it happens
Sparsentan blocks efferent arteriolar tone while NSAIDs inhibit prostaglandin-mediated afferent dilation, together sharply reducing glomerular filtration.
What to do
Avoid regular NSAID use in patients on sparsentan and choose an alternative analgesic such as paracetamol. If unavoidable, monitor serum creatinine, potassium and blood pressure closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
