Summary

Opioids slow gastric emptying, which delays and lowers absorption of oral prasugrel and the formation of its active metabolite. Platelet inhibition may therefore be delayed or reduced in the early period after a loading dose, which matters in acute coronary syndrome and PCI.

What this interaction means in plain words

Fentanyl and Prasugrel can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fentanyl belongs to Musculoskeletal; Prasugrel belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Opioid-induced delay in gastric emptying reduces and slows the absorption of prasugrel's active metabolite.

What to do

In acute coronary syndrome patients who need an opioid, consider a parenteral antiplatelet agent instead of relying on oral prasugrel. Otherwise monitor for inadequate antiplatelet effect after the loading dose.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
