Summary
Opioids such as fentanyl may promote antidiuretic hormone release and urinary retention, which could blunt the diuretic effect of hydrochlorothiazide. Opioids can also add to the blood pressure lowering of antihypertensives, particularly at initiation or dose increases.
What this interaction means in plain words
Fentanyl transmucosal and Fosinopril-hydrochlorothiazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Opioid-related ADH release and urinary retention may oppose diuresis, while additive pharmacodynamic hypotensive effects may occur.
What to do
Monitor blood pressure, fluid status and urine output when starting or increasing fentanyl. Adjust antihypertensive or diuretic doses if hypotension or fluid retention develops.
