Summary

Fexinidazole may inhibit CYP3A4 and could modestly raise exposure to CYP3A4 substrates. Losartan is converted to its active metabolite partly by CYP3A4 and CYP2C9, so the net effect on blood pressure is unpredictable and likely small. Fexinidazole also prolongs the QT interval, but losartan and hydrochlorothiazide do not share that effect, apart from electrolyte disturbance from the thiazide.

What this interaction means in plain words

Fexinidazole and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Possible CYP3A4 inhibition by fexinidazole altering losartan activation; thiazide-induced hypokalemia or hypomagnesemia may add QT risk.

What to do

Monitor blood pressure and check potassium and magnesium before and during fexinidazole treatment, correcting abnormalities.

What to ask your pharmacist
