Glasdegib can prolong the QT interval, and fingolimod causes bradycardia and may affect QT. The combination increases the risk of ventricular arrhythmia including torsades de pointes.
What this interaction means in plain words
Fingolimod and Glasdegib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fingolimod belongs to Central Nervous System; Glasdegib belongs to Oncology.
Why it happens
Additive QTc prolongation, with fingolimod-induced bradycardia increasing arrhythmia susceptibility.
What to do
Obtain a baseline ECG and electrolytes, and monitor ECG regularly during co-treatment. Correct potassium and magnesium, and interrupt glasdegib if the QTc becomes markedly prolonged.
