Summary
Systemic corticosteroids can blunt the expansion and activity of CAR-T cells and add to infection risk in a patient who is already immunocompromised. Fludrocortisone has mainly mineralocorticoid activity, so the effect is likely smaller than with glucocorticoids.
What this interaction means in plain words
Fludrocortisone and Idecabtagene vicleucel can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fludrocortisone belongs to Endocrine; Idecabtagene vicleucel belongs to Oncology.
Why it happens
Corticosteroid-related lymphocyte suppression may reduce CAR-T cell proliferation and persistence and adds to immunosuppression.
What to do
Avoid corticosteroid premedication before CAR-T infusion unless needed for adrenal replacement. Patients on fludrocortisone for adrenal insufficiency should continue it, and corticosteroids may still be used for cytokine release syndrome or neurotoxicity.
