Summary

Concurrent corticosteroid and fluoroquinolone use raises the risk of tendinitis and tendon rupture. Fludrocortisone has mainly mineralocorticoid activity at usual doses, so the risk is probably lower than with glucocorticoids, but it still applies.

What this interaction means in plain words

Fludrocortisone and Ofloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fludrocortisone belongs to Endocrine; Ofloxacin belongs to Anti Bacterial.

Evidence level: Low

Why it happens

Corticosteroids and fluoroquinolones both impair tendon collagen turnover, producing additive tendon toxicity.

What to do

Counsel the patient to report tendon pain or swelling promptly and stop ofloxacin if this occurs. Consider a non-quinolone antibiotic in older patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
