Summary
Fluoxetine can prolong the QT interval and beta2-agonists such as levosalbutamol can cause tachycardia, hypokalemia and QT prolongation at higher doses. Together they may add to the risk of arrhythmia, particularly with high doses, hypokalemia or other risk factors.
What this interaction means in plain words
Fluoxetine and Levosalbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluoxetine belongs to Central Nervous System; Levosalbutamol belongs to Respiratory.
Why it happens
Additive QT prolongation from fluoxetine and beta2-agonist effects, with beta2-induced hypokalemia increasing susceptibility.
What to do
Monitor ECG and serum potassium in patients with cardiac disease, electrolyte disturbance or other QT-prolonging drugs. Avoid excessive levosalbutamol dosing and correct hypokalemia.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
