Summary
Fluoxetine can prolong the QT interval, and beta2 agonists such as olodaterol can cause tachycardia, hypokalemia and QT effects. Together they may add to the risk of arrhythmia, particularly in patients with cardiac disease or electrolyte disturbances. Systemic exposure to inhaled olodaterol is low, so the risk is mostly theoretical at usual doses.
What this interaction means in plain words
Fluoxetine and Olodaterol inhaled have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Additive pharmacodynamic effects on cardiac repolarization and heart rate, with possible beta2-agonist-induced hypokalemia.
What to do
Correct potassium and magnesium before starting and monitor heart rate and ECG in patients with QT risk factors. Avoid exceeding the recommended olodaterol dose.
