Summary
Amprenavir, the active metabolite of fosamprenavir, inhibits CYP3A4 and may modestly increase prednisolone exposure, raising the risk of corticosteroid adverse effects with prolonged use. Corticosteroids may in turn lower amprenavir levels slightly.
What this interaction means in plain words
Fosamprenavir and Prednisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of CYP3A4 by amprenavir (boosted by ritonavir if present) reduces prednisolone clearance.
What to do
Monitor for Cushingoid effects, hyperglycemia and adrenal suppression with long-term use, and use the lowest effective prednisone dose. Consider an alternative less CYP3A4-dependent steroid for prolonged therapy with ritonavir-boosted regimens.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
