Summary

Thiazides may reduce the renal tubular secretion of ganciclovir, and ganciclovir is cleared mainly by the kidney. In addition, volume depletion from the diuretic can lower renal function and further raise ganciclovir exposure, increasing the risk of myelosuppression.

What this interaction means in plain words

Fosinopril-hydrochlorothiazide and Ganciclovir have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Ganciclovir belongs to Anti Virals.

Evidence level: Low

Why it happens

Possible competition for renal tubular secretion plus diuretic-related reduction in renal function.

What to do

Monitor renal function and blood counts during ganciclovir therapy, and adjust the ganciclovir dose to creatinine clearance.
