Summary

Heparin suppresses aldosterone secretion and, with an ACE inhibitor, can lead to hyperkalemia, particularly in patients with renal impairment or diabetes or with prolonged therapy. The thiazide partly offsets this.

What this interaction means in plain words

Fosinopril-hydrochlorothiazide and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Heparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced inhibition of aldosterone synthesis adds to ACE inhibitor-mediated potassium retention.

What to do

Monitor serum potassium when heparin is continued for more than a few days or in high-risk patients. Treat hyperkalemia and reassess therapy if it occurs.
