Summary
ACE inhibitors may slightly improve glycemic control, which can occasionally lead to lower glucose in patients treated with metformin. Thiazides can raise glucose and, with ACE inhibitors, may reduce renal function, which would increase metformin accumulation and the risk of lactic acidosis.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Metformin belongs to Endocrine.
Why it happens
Altered insulin sensitivity from ACE inhibition, plus potential reduction in renal function that decreases metformin clearance.
What to do
Monitor glucose and renal function regularly, and reassess metformin dosing if eGFR declines. Hold metformin during acute illness with dehydration.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
