Summary
ACE inhibitors reduce aldosterone and potassium excretion, so potassium phosphate supplements can raise serum potassium, sometimes to dangerous levels. The thiazide component partly offsets this, but the risk remains, especially in renal impairment or diabetes.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Potassium acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
ACE inhibition reduces aldosterone-mediated renal potassium excretion, adding to the potassium load from the supplement.
What to do
Check serum potassium and creatinine at baseline and within the first week or two, then periodically. Reduce or stop the potassium supplement if potassium rises above the normal range.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
