Summary
Potassium citrate supplies absorbable potassium, and ACE inhibitors limit its renal elimination, so serum potassium can rise. The risk is greater in renal impairment, diabetes or with other potassium-raising drugs, while the thiazide partly counteracts it.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Potassium citrate / citric acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
ACE inhibition reduces aldosterone-dependent potassium excretion, adding to the exogenous potassium load.
What to do
Monitor serum potassium and creatinine at the start and periodically afterward. Reduce the potassium dose if hyperkalemia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
