Summary
Potassium phosphate supplements add a potassium load, and ACE inhibitors reduce renal potassium excretion, so hyperkalemia can develop. The thiazide partially offsets this, but risk remains high in renal impairment or with intravenous administration.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Potassium phosphates can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
ACE inhibition lowers aldosterone-mediated potassium excretion while the supplement increases potassium intake.
What to do
Monitor serum potassium and renal function closely and use cautious dosing and infusion rates; ECG monitoring is advisable with IV potassium.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
