Summary

Non-acetylated salicylates can reduce prostaglandin-dependent renal blood flow, which may blunt the antihypertensive effect of ACE inhibitors and diuretics and worsen renal function in volume-depleted or elderly patients. The effect is generally smaller than with conventional NSAIDs.

What this interaction means in plain words

Fosinopril-hydrochlorothiazide and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of renal vasodilatory prostaglandins reduces antihypertensive efficacy and glomerular perfusion.

What to do

Monitor blood pressure, renal function and potassium, and ensure adequate hydration. Use the lowest effective salicylate dose or switch to paracetamol if analgesia alone is needed.

What to ask your pharmacist
