Summary
Bowel preparations cause fluid and electrolyte shifts and diarrhea, and thiazide diuretics add to volume and electrolyte depletion. The ACE inhibitor raises the risk of acute kidney injury when volume depleted. This combination can cause dehydration, electrolyte disturbances and kidney injury.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Sodium sulfate / potassium chloride / magnesium sulfate / polyethylene glycol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive fluid and electrolyte losses with thiazide diuresis, and impaired renal autoregulation from ACE inhibition during volume depletion.
What to do
Consider withholding the diuretic and ACE inhibitor on the day of bowel preparation, and ensure adequate hydration. Check electrolytes and renal function in high-risk patients.
