Summary

Fosphenytoin (phenytoin) induces CYP2C8, CYP3A4 and P-glycoprotein, which can lower paclitaxel exposure and possibly reduce antitumor efficacy. Paclitaxel itself may also alter phenytoin levels, which can affect seizure control.

What this interaction means in plain words

Fosphenytoin and Paclitaxel can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fosphenytoin belongs to Central Nervous System; Paclitaxel belongs to Oncology.

Evidence level: Moderate

Why it happens

Phenytoin induces CYP2C8/CYP3A4 and P-glycoprotein, increasing paclitaxel clearance.

What to do

Where possible choose a non-enzyme-inducing antiseizure drug such as levetiracetam; if continued, monitor phenytoin levels and treatment response.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
