Summary
Ritonavir may lower phenytoin concentrations through enzyme induction, risking loss of seizure control. Phenytoin may also lower ritonavir exposure, which can reduce antiretroviral efficacy.
What this interaction means in plain words
Fosphenytoin and Ritonavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fosphenytoin belongs to Central Nervous System; Ritonavir belongs to Anti Virals.
Why it happens
Induction of CYP2C9 and CYP3A4 (and glucuronidation) lowers concentrations of both drugs, though ritonavir also inhibits some pathways, so the net effect is variable.
What to do
Monitor phenytoin levels and seizure control, and adjust the dose accordingly. Monitor HIV viral load and ritonavir exposure, or choose an alternative anticonvulsant.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
