Summary

Fostemsavir's active metabolite temsavir inhibits the OATP1B1/3 and BCRP transporters, which take part in glyburide uptake and efflux. Exposure to glyburide may therefore rise, increasing the likelihood of hypoglycemia. The effect is probably modest in most patients.

What this interaction means in plain words

Fostemsavir and Glibenclamide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Fostemsavir belongs to Anti Virals; Glibenclamide belongs to Endocrine.

Evidence level: Low

Why it happens

Temsavir inhibits OATP1B1/3 and BCRP transporters, reducing hepatic uptake and efflux of glyburide.

What to do

Monitor blood glucose when fostemsavir is started or stopped in a patient taking glyburide. Reduce the glyburide dose if hypoglycemia occurs.
