Summary

Both drugs inhibit acetylcholinesterase, so combining them can amplify cholinergic effects such as bradycardia, nausea, vomiting, diarrhea, salivation and bronchospasm. The risk of cholinergic toxicity is greater at higher doses or in patients with cardiac conduction disease or asthma.

What this interaction means in plain words

Galantamine and Neostigmine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Galantamine belongs to Central Nervous System; Neostigmine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic cholinesterase inhibition leading to increased acetylcholine at muscarinic and nicotinic receptors.

What to do

Avoid routine duplication of cholinesterase inhibitors; if used together, monitor heart rate and for GI and respiratory cholinergic symptoms and reduce the dose if they appear.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
