Summary

Glecaprevir and pibrentasvir inhibit P-gp, BCRP and OATP1B1/1B3, which may increase glyburide exposure and the risk of hypoglycemia. Glyburide is itself an OATP substrate and inhibitor, so reciprocal effects are possible but less important. Improved glycemic control after hepatitis C cure may also lower glucose requirements.

What this interaction means in plain words

Glecaprevir / pibrentasvir and Glyburide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of intestinal and hepatic transporters (P-gp, BCRP, OATP) by glecaprevir/pibrentasvir raises glyburide concentrations.

What to do

Monitor blood glucose during and after hepatitis C therapy and reduce the glyburide dose if hypoglycemia develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
