Summary
Olanzapine commonly causes weight gain, insulin resistance and hyperglycaemia, which can reduce the effect of glimepiride. Blood glucose control may worsen after olanzapine is started or increased, and may need escalation of diabetes treatment.
What this interaction means in plain words
Glimepiride and Olanzapine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Glimepiride belongs to Endocrine; Olanzapine belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism: olanzapine-induced insulin resistance and impaired insulin secretion oppose sulfonylurea action.
What to do
Monitor fasting glucose and HbA1c at the start of and during olanzapine therapy and adjust glimepiride or add therapy as needed. Consider a more metabolically neutral antipsychotic if control is difficult.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
