Summary

Fluoroquinolones can cause both severe hypoglycemia and hyperglycemia in patients taking antidiabetic drugs, especially sulfonylureas. Older and renally impaired patients are at greater risk.

What this interaction means in plain words

Glipizide-metformin and Ofloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Glipizide-metformin belongs to Endocrine; Ofloxacin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Fluoroquinolones can stimulate pancreatic insulin release by blocking beta-cell potassium channels, adding to sulfonylurea action; the cause of hyperglycemia is less clear.

What to do

Monitor blood glucose during the antibiotic course and educate the patient about hypoglycemia symptoms. Stop the fluoroquinolone and treat if significant dysglycemia occurs.
