Summary
Metformin and quinine are both cationic drugs eliminated by renal tubular secretion, and they may compete for transporters. This can modestly raise quinine or metformin exposure. Quinine itself can also cause hypoglycemia, which may add to the effect of glipizide.
What this interaction means in plain words
Glipizide-metformin and Quinine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Glipizide-metformin belongs to Endocrine; Quinine belongs to Anti Parasitic.
Why it happens
Competition for renal tubular cation transporters (OCT2/MATE), plus additive hypoglycemic effect of quinine through insulin secretion stimulation.
What to do
Monitor blood glucose during quinine therapy and watch for quinine adverse effects such as cinchonism. Adjust the sulfonylurea dose if hypoglycemia occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
