Summary

Heparin can suppress aldosterone synthesis and cause hyperkalemia. Combined with losartan, which also reduces aldosterone effect, the risk of raised potassium is increased, particularly in renal impairment or diabetes.

What this interaction means in plain words

Heparin and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Heparin belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive suppression of aldosterone-mediated potassium excretion by heparin and angiotensin receptor blockade.

What to do

Check serum potassium and renal function at baseline and after several days of heparin. The thiazide partly offsets the risk, but monitoring is still needed.

What to ask your pharmacist
