Summary

Both drugs lower blood pressure, and lofexidine also slows heart rate. Combined use may cause excessive hypotension and bradycardia, although hydralazine's reflex tachycardia may partly offset the bradycardia.

What this interaction means in plain words

Hydralazine and Lofexidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hydralazine belongs to Cardiovascular; Lofexidine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive hypotensive effect from direct arteriolar vasodilation and central alpha-2 sympatholysis.

What to do

Monitor blood pressure and pulse, especially during lofexidine initiation and dose changes, and reduce doses if symptomatic hypotension occurs.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
