Summary

Both levalbuterol and hydrochlorothiazide can lower serum potassium. Together they may cause clinically significant hypokalemia, particularly with high-dose or frequent bronchodilator use, with a risk of arrhythmia. Losartan may partly offset this by raising potassium.

What this interaction means in plain words

Hydrochlorothiazide-losartan and Levalbuterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Beta-2 agonists shift potassium into cells while thiazides increase renal potassium loss, producing additive hypokalemia.

What to do

Monitor serum potassium in patients using frequent or high-dose levalbuterol, and replace potassium as needed.

What to ask your pharmacist
