Summary
Non-acetylated salicylates have weaker prostaglandin inhibition than other NSAIDs, but at higher doses they may still blunt the antihypertensive effect of losartan and hydrochlorothiazide. Renal function can decline in volume-depleted or elderly patients, and potassium balance may change.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Reduced synthesis of vasodilatory renal prostaglandins may impair renal perfusion and sodium excretion, opposing the effects of the angiotensin receptor blocker and thiazide.
What to do
Monitor blood pressure, serum creatinine and potassium when salicylates are used regularly at analgesic or anti-inflammatory doses. Ensure adequate hydration and use the lowest effective salicylate dose.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
