Summary

Vilanterol, a long-acting beta-agonist, can lower serum potassium, and hydrochlorothiazide also causes potassium loss. Together they may cause hypokalemia and ECG changes, particularly if the beta-agonist dose is exceeded; losartan partly offsets potassium loss.

What this interaction means in plain words

Hydrochlorothiazide-losartan and Umeclidinium bromide / vilanterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive hypokalemia from beta-2 stimulated intracellular potassium shift and thiazide renal potassium loss.

What to do

Monitor serum potassium in patients with risk factors such as heart disease or high inhaler doses. Correct hypokalemia and keep the inhaler at the labeled dose.
