Summary
Olmesartan reduces aldosterone-mediated potassium excretion, so potassium supplements can cause hyperkalemia, while hydrochlorothiazide tends to lower potassium and partly offsets this. Net potassium effect is unpredictable, and hyperkalemia is more likely in patients with renal impairment or diabetes.
What this interaction means in plain words
Hydrochlorothiazide-olmesartan and Potassium citrate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-olmesartan belongs to Cardiovascular; Potassium citrate belongs to Gastrointestinal Tract.
Why it happens
ARB-induced reduction of renal potassium excretion combined with potassium supplementation, partly offset by thiazide-induced potassium loss.
What to do
Check serum potassium and renal function before and shortly after starting potassium citrate, then periodically. Adjust or stop the supplement if potassium rises.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
