Summary
Olmesartan reduces aldosterone-mediated potassium excretion, so added potassium supplementation can lead to hyperkalemia. Hydrochlorothiazide has a potassium-wasting effect that partly offsets this, but the net effect is unpredictable, particularly in renal impairment.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Potassium citrate can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Potassium citrate belongs to Gastrointestinal Tract.
Why it happens
ARB-induced aldosterone suppression decreases renal potassium excretion, adding to the potassium load from the supplement.
What to do
Check serum potassium and renal function at baseline and shortly after starting, then periodically. Avoid or reduce the supplement if potassium rises or eGFR is reduced.
