Summary
Quinidine competes for renal tubular secretion and could modestly alter hydrochlorothiazide exposure. Thiazide-induced hypokalemia or hypomagnesemia also predisposes patients to quinidine-related QT prolongation and torsades de pointes.
What this interaction means in plain words
Hydrochlorothiazide-olmesartan and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-olmesartan belongs to Cardiovascular; Quinidine belongs to Anti Parasitic.
Why it happens
Thiazide-induced electrolyte depletion increases proarrhythmic risk of quinidine, with possible competition for tubular cation transport.
What to do
Monitor serum potassium and magnesium and correct deficiencies. Obtain an ECG to monitor the QT interval.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
