Summary

Quinine is partly cleared by the kidney, so competition with thiazides for tubular secretion is theoretically possible. Thiazide-induced hypokalemia or hypomagnesemia could also increase the risk of QT prolongation with quinine.

What this interaction means in plain words

Hydrochlorothiazide-telmisartan and Quinine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Hydrochlorothiazide-telmisartan belongs to Cardiovascular; Quinine belongs to Anti Parasitic.

Evidence level: Low

Why it happens

Theoretical competition for renal tubular cationic secretion; electrolyte disturbance may add to quinine's QT effect.

What to do

Check potassium and magnesium and correct deficits before and during quinine therapy. Consider an ECG in patients with other QT risk factors.
