Summary
Ibuprofen can blunt the antihypertensive effect of valsartan and hydrochlorothiazide. In combination with a diuretic and an angiotensin receptor blocker it also raises the risk of acute kidney injury, especially in older or volume-depleted patients, and may cause hyperkalemia.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Ibuprofen can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Hydrochlorothiazide-valsartan belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins reduces afferent arteriolar dilation and sodium excretion, which together with angiotensin receptor blockade and diuretic-induced volume loss reduces glomerular filtration.
What to do
Avoid regular NSAID use where possible and use paracetamol or another alternative. If it cannot be avoided, keep the dose and duration low, ensure hydration, and monitor blood pressure, creatinine and potassium.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
